Page 11, top of second column.

You always choose the order in which events that affect your creatures and objects occur during this phase. In the rare case that a timing issue occurs, the player with the initiative decides the order.

This seems to be a contridiction...

If the Unicorn question (burn vs. regen) isn't an example of a timing issue... what is?

I was thinking about timing issue:
Player A has Malacoda, and Malacoda is in the same zone as player B's creatures. Malacoda damages these (living) creatures at the Upkeep Phase. B can choose to regenerate first or take damage first, because these affecting his creatures.

Let's say Malacoda has an Upkeep +X trait (from enchantment). Maybe A doesn't want to pay the mana. I think here comes the Initiative thing.

If A has the Initiative he can say first Malacoda damages and after he pay or not the mana. B still can decide to use regen (if there are any) before of after the damage.

If B has the Initiative he can force A to decide to pay the mana before Malacoda damages the creatures.

Am I right?

I believe you are correct, another situation with timing issues:

Wizard only has 1 health before dying, has regen, and at least 1 curse.
Warlock has Moloch's Torment.

If Wizard has Initiative, then he can heal first and survive the 1 damage from Warlock.
If Warlock has Initiative, then he can damage Wizard first, thus winning the game.
