When two mandatory enchantments are triggered at the same time (such as casting an incantation on someone with both Nullify and Reverse Magic enchanting him) are both of them revealed, even if the trigger no longer applies after one is resolved? Is the answer the same in the case of traps, when the first trap kills (or teleports away, etc) the creature that triggered the trap?

The answer is yes to both questions.
You must reveal both enchantments, then you decide if you want to pay the reveal cost of one of the enchantments, both, or neither. Of course in the case you mentioned there would be no benefit to paying for both of them.

Thank you! So, just to be clear, in the case of two traps, you would reveal both, then decide which one(s) you want to pay the reveal cost for before they even resolve? For example, a Hellfire Trap and a Teleport Trap. Obviously if you're paying the cost on both, you would choose to resolve the Hellfire Trap first. Do you decide whether or not to pay the reveal cost on the Teleport Trap before you resolve the Hellfire Trap?

You would decide before the Hellfire Trap is resolved. You decide whether or not your going to pay the reveal cost when the trap is revealed. Then you choose the order you resolve them. So in the example you gave you would roll the damage from the Hellfire Trap then if the creature was still alive teleport it with the Teleport Trap if you payed its reveal cost.
