When you cast an equipment spell with spellbinding and bind another spell to it, do both cards come from the two you chose for that round, or is the bound spell a third 'freebie'?

Freebie.

"When you cast a spell with the Spellbind trait, you may immediately bind a spell to it. You can choose any spell from your spellbook that fulfills the limitations on the Spellbind object."

Do you have to pay the mana to bind the spell or only pay the mana when you cast the spell as an attack etc

Binding the spell costs no mana, but changing the bound spell to another one is a 3 mana quick spell.
